Q: How do I make an appointment?

a: Please check with your insurance carrier to determine if a referral or authorization is needed. If so, please contact your primary care doctor. Afterwards, please contact our office to schedule an appointment.
If you are being seen for workers’ compensation or auto insurance, please have your adjuster contact our office.
If you have any questions, please feel free to contact us directly.

Q: What items are needed for my appointment?

a: All new patients are required to have diagnostic imaging (films and reports) that has been completed within the last 6 -12 months.
a: Completed new patient paperwork.
a: Insurance card & photo ID

Q: Can I get an appointment in the evening?

a: Yes. We do offer select appointments on Thursday evening between 5:00pm and 8:00pm.

Q: How quickly can I be scheduled for an appointment?

a: Typically, most new patients are scheduled to be seen within one to weeks from the time they call. Patients desiring to be seen sooner can be placed on our cancellation list to be contacted if an opening arises.

Q: What time do I need to arrive for my appointment?

a: Please arrive 15 minutes early for your appointment to assist us in ensuring you have all necessary items in order to be seen.

Q: How long will my appointment take?

a: Appointment times can vary greatly depending upon the requested testing, physical examination and follow up care ordered. Please allow a minimum of one hour for follow up appointments and two hours for new patient appointments. We strive to see you at your scheduled appointment time. In the “real world” occasional unavoidable delays occur, and emergencies do arise.

Q: What notice do I need to give you when cancelling my appointment?

a: If you’re unable to keep your scheduled appointment, please let us know 24 hours in advance, or earlier, so that we can make that time available to someone else. If we are not allowed sufficient time to rebook the appointment, you may be charged a fee for cancellation.

Q: Will you see me for a second opinion?

a: Yes, a second opinion is similar to a new patient appointment. We do ask that you provide us with a copy of the initial evaluation for review.
