2. Do you accept workers’ compensation?
- Yes; we accept Florida, out of state, and Federal workers’ compensation. Your adjuster or case manager should contact us to schedule an appointment.
3. Do you accept personal injury/ automobile insurance?
- Yes, we accept personal injury/automobile insurance. However, we do require patients to also have commercial insurance as secondary coverage.
4. Will you accept a letter of protection?
- Our office accepts letters of protection on a case by case basis pending physician review. Please contact our office for more information.
5. How much do I need to pay at the time of my appointment?
- We expect payment at the time of service. Depending upon your insurance coverage, this amount may vary depending upon co-pays, co-insurance and deductibles. Please check with your insurance carrier.
6. How do I know what my co-pay is?
- Some patients have a co-pay for services. If your insurance works in this manner, your co-pay will be indicated on your insurance card or you will have a number to call listed.
7. What is co-insurance and how do I know if I have it?
- Co-insurance is where patients are responsible for a percentage of the cost of their visit. For example, a patient may be responsible for 20% of the cost of the office visit. Please contact your insurance carrier for further details.
8. How does my deductible work with your office?
- A deductible is a flat amount that must be paid prior to an insurance carrier covering a patient’s care. For example, a patient’s deductible could be $1,000. The patient is responsible for paying the first $1,000 of their medical care, after which insurance picks up the bills. This may result in a patient having to cover the cost of part or the entire visit to our office. Please contact your insurance carrier for further details.
9. Do I need to bring my insurance card to my appointment?
- Yes, we need to make a copy of your insurance card and verify your coverage.
10. What if I have a past due balance?
- We expect payment at the time of service. Any patient with a past due balance must provide payment in full prior to being seen. However, as we are aware that circumstances may arise, we do offer a payment plan to qualified individuals.
11. Can I be seen if I don’t have insurance?
- Patients without insurance may be seen on a self-pay basis. Payment is due at the time of service and an estimate may be given in advance. Please contact our billing department for pricing.
