1. How do I get a copy of my records?

- You should send us a written request by postal service, email or fax.

2. Is there a fee for medical records?

- Patients are provided a one-time copy of their record at no charge. Subsequent copies are provided at a per page rate. All legal requests for records are charged at a per page rate.

3. How long does it take to process a request for records?

- Processing times may vary depending upon the status of the record requested. Typically, we request a minimum of seven days for processing.

4. What do I need to do for my family doctor or specialist to obtain my records?

- We are happy to provide records for your continued medical care at no cost to any physician actively treating you. We ask that you provide us with the name of the physician and verify their office information. In certain instances, we may request a written authorization signed by you. We are only able to provide records to two providers on an ongoing basis, however we will gladly provide additional records upon request.

5. What do I need to do for my significant other/ family member to obtain my information?

- This information should be included on the Consent form that you complete at the time of your new patient appointment. You are welcome to update this form at any time.
